Abstract
Introduction: Excess body weight alters the sagittal balance due to compensatory mechanisms in the spine and pelvis, promoting biomechanical changes. Determine spinopelvic parameters in overweight and obese adult patients at the National Medical Center (CMN) in southeastern Mexico. Methods: A total of 150 adult patients were included. Lateral lumbar spine radiographs were examined to measure lumbar lordosis (LL), pelvic incidence (PI), pelvic tilt (PT), and sacral slope (SS). Statistical analysis included normality tests, Chi-square tests for associations between body mass index (BMI) and spinopelvic parameters, ANOVA for comparing means according to obesity grade, and Pearson and Spearman correlation analyses. Results: A female predominance was observed. PI showed a statistically significant increase correlated with elevated BMI (mean PI up to 74.97° in grade 3 obesity, p=0.02). PT showed a tendency to increase in obese patients, without statistical significance. No significant changes in LL or SS were observed according to weight status. Significant associations were documented between age and overweight/obesity (p=0.010) and between age and PI and PT (p<0.05). Significant correlations were found between LL with PI and SS, and between PT with PI and SS (with a negative correlation with SS). Conclusion: The findings confirm that overweight and obesity are associated with modifications in spinopelvic parameters, particularly PI and PT, suggesting alterations in spinal sagittal balance. Pelvic incidence and pelvic tilt are significantly modified in overweight and obese patients, correlating with age and degree of obesity.
